Personal information on this form is collected under the authority of section 10(1) of the Municipal Act, 2001, as amended and will be used solely to contact persons applying for a Gymnasium Permit with the City of Brantford. All organizations/permit holders must provide a minimum $2 million or $5 million dollars (based on activity) liability insurance naming the Corporation of the City of Brantford as “Additional Insured”. Events and activities deemed high risk may require higher coverage. Liability insurance is available to purchase for individuals/leagues that do not have access to other insurance, (e.g. governing body insurance). The insurance premium will be added to the permit.
